PDS Return Process Guidelines
If you are experiencing difficulties with your Precision Delivery System, please follow steps 1 through 4.
Follow these steps:
- FIRST: Please CLICK HERE to review the PDS Troubleshooting Guide before submitting a PDS Service Request Form.
- If your device is still malfunctioning, DOWNLOAD and fill out the PDS Service Request Form**
- EMAIL the completed form to firstname.lastname@example.org and carbon copy your CSI sales rep and your Distributor sales rep.
- PRINT AND PLACE the completed form in a box with the defective PDS (for shipping purposes, place the defective PDS in a plastic bag inside the box).
Control Solutions will have the defective PDS picked up from the location indicated on the form.
**NOTE: If the PDS Service Request Form is not filled out completely, it will delay the process!
